We recommend that you spend a few moments familiarizing yourself with your medical benefits before your appointment.

Our dedicated staff will do everything possible to see that you receive the full benefits of your policy and will file claims directly to
your insurance plan.

While we intend to assist you, your policy is a contract between you and your insurance plan, and all charges are ultimately the patient’s responsibility. We cannot become involved in disputes between you and your insurer regarding deductibles, co-pays, covered costs, medical necessity, or “usual and customary fees.” If your insurance company has not paid your claim within 45 days, any balance will be billed to your account. Please be aware that some services, exams, or lab work performed, may not be covered under your plan. If you are uncertain, please contact our office.

Please be prepared to pay all co-pays, deductibles, and co-insurance in full, at the time of your visit. If we are unable to verify medical benefits before your appointment, you may be asked to pay all charges at your visit.

Please inform our office of any changes in your coverage, address, or contact numbers.
